Since 2016, the Centre of Excellence on Longevity has created and operates an Internet platform for health professionals to improve the care pathway for seniors and family caregivers.
This platform, called CESAM, will be accessible to the general public throughout Quebec in the spring of 2019.
The family caregiver must be identified, accompanied and followed up. To do this, he must himself consider that he needs help and get involved in his own health.
In this perspective, the Centre of Excellence on Longevity will make CESAM available free of charge to family caregivers of all ages, who will thus have access to simple and scientifically validated tools and questionnaires to assess their own quality of life and state of health. CESAM will make it possible to identify family caregivers, establish their socio-demographic profile, assess their needs and give the keys to better support them and help them to help.
